Admissions Registered Nurse, Tues-Fri noon to 10pm

Overview Admissions Nurse — Hours: Tuesday–Friday days, noon to 10pm. Ascend Hospice is part of the Ascend family of Healthcare Companies. Responsibilities Completes an initial assessment of patient and family to determine home care needs and identifies problems for each patient upon admission. Ability to outline the patient/family referral and admission. Uses assessment data to determine care plan needs and initiates the plan of care. Articulates hospice philosophy, goals and objectives. Communicates with other members of the clinical team to coordinate the care plan. Markets to acute hospitals in the area. Position Details The Admissions Nurse will be responsible for but not limited to: completing an initial assessment, outlining referrals, initiating care plans, and coordinating with the clinical team. Education and Experience Associate's Degree in Nursing required, Bachelor's Degree preferred. 2-4 years of related Hospice experience. Licensed as a Registered Nurse within the state of operation.
