Physical Therapist – Home HealthCompanion Home Health is an Oklahoma-owned, Medicare-certified agency. We help patients recover at home after illness, injury, or surgery. We are hiring a Physical Therapist to provide skilled therapy in patients’ homes across central Oklahoma. This role is paid per visit, with an expected annual range of $85,000–$95,000 for a full caseload. You will receive a company car and gas card, so you are not putting miles on your personal vehicle.What We Offer$85,000–$95,000 annual earning range, paid per visitCompany car and gas cardIndependent, field-based scheduleSupport from nursing, therapy, and case managementOpportunity to work with patients and families in their own homesWhat You Will DoProvide skilled physical therapy according to the physician’s plan of treatmentAccept only clients referred by the Director of Nursing or designeeAssess function, apply appropriate tests, and help develop and update the plan of careTreat patients to relieve pain, restore function, and maintain the highest possible performance using the treatments and modalities in the plan of careRe-evaluate regularly, revise the plan of care as needed, and document why a identified problem is not addressed when that is the caseStart appropriate diagnostic, preventive, and rehabilitative procedures and refer to other disciplines when the patient’s needs require itObserve changes in condition and report them promptly to the physician and/or RN/Case ManagerTeach patients and families home exercise programs, safety, use of prosthetic/orthotic devices, and durable medical equipmentCoordinate care with nursing and other disciplines and attend care conferences as neededComplete accurate documentation within 24 hours of the visitParticipate in quality improvement, in-services, and ongoing professional educationProvide staff education when requested and arranged by the RN/Case ManagerSchedule and Work SettingFlexible field schedule with travel to patient homes and community appointmentsService area includes Oklahoma City, Edmond, Guthrie, Stillwater, Perry, and surrounding communitiesWork is performed in patient homes and in the community in a range of weather and home environmentsRequirementsGraduate of a physical therapy program approved by CAPTE / APTA (or equivalent approved program)Current Oklahoma Physical Therapist licenseCurrent CPRValid driver’s licenseAbility to work independently and as part of an interdisciplinary teamStrong verbal, written, and interpersonal communication skillsAbility to work a flexible schedulePreferredOne year of clinical experience, ideally in home healthPhysical RequirementsThis role requires travel between homes and the ability to provide hands-on therapy in varied home settings. That typically includes standing, walking, stooping, kneeling, reaching, and assisting patients. You may lift, carry, push, or pull about 10–15 lbs. intermittently.
