Primary Care Physician AssistantKaiser Permanente-Washington Permanente Medical Group is seeking a full-time 1.0 FTE Primary Care Physician Assistant to join our Quality, Documentation & Reporting Team at our Tacoma Medical Center in Tacoma, Washington. This clinical role will focus on Co-visit Care for Chronic Conditions (CC3).The CC3 (Co-visit Care for Chronic Conditions) Clinician will work to ensure quality care for identified WPMG patients utilizing virtual encounters to address conditions and close care gaps in a patient centered manner. Whenever possible, they will preview, update, and modify the health records in HealthConnect to optimize the chart for their encounters. The work completed by the CC3 Clinicians will be monitored through proficiency metrics and chart audits to ensure quality of documentation.
