Position Overview

We are seeking a highly organized, personable, and proactive Care Manager to join our growing team in our Howell, NJ office.

As a Care Manager, you will become the primary administrative point of contact for each client after the intake process is completed. From that point forward, you will oversee the client's ongoing care journey, ensuring they receive consistent, high-quality services while maintaining excellent communication with families, clinicians, and internal departments.

This role is ideal for someone who enjoys solving problems, building relationships, and keeping many moving pieces organized in a fast-paced environment.

Responsibilities

• Take ownership of each client immediately following the intake process.

• Manage the client throughout the entire lifecycle of services.

• Coordinate and maintain client schedules.

• Staff clients with the appropriate Behavior Technician (BT) and Board Certified Behavior Analyst (BCBA).

• Quickly resolve scheduling conflicts, cancellations, and coverage needs.

• Communicate daily with families regarding schedules, updates, and ongoing support.

• Collaborate closely with BCBAs, BTs, Intake, Recruiting, Authorization, and Leadership teams to ensure seamless service delivery.

• Monitor client schedules to maintain continuity of care.

• Identify potential issues before they impact services and proactively develop solutions.

• Maintain accurate documentation and utilize company software to manage client information and workflows.

• Help create a positive experience for both our families and our clinical staff.

Qualifications

• Excellent communication and interpersonal skills.

• Strong organizational and multitasking abilities.

• Ability to remain calm and solution-oriented in a fast-paced environment.

• Comfortable learning new technology and software systems.

• Ability to work collaboratively within a team.

• Previous experience in healthcare, ABA, care coordination, scheduling, or case management is preferred but not required.

• Experience with Monday.com and CentralReach is a plus.
