Job Title: Care Manager Profession: RN Specialty: Case Management Duration: 13 weeks Shift: Days Hours per Shift: 36 Experience: Two (2) years of health care experience as a Registered Nurse License: Licensed to practice as a Registered Nurse in the state of North Carolina Certifications: N/A Must-Have: Strong assessment and critical thinking skills Description: The purpose of this position is to provide ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs. The overall goal of the position is to enhance the quality of patient management and satisfaction. This role promotes continuity of care and cost-effectiveness through the integration of case management, utilization review, and discharge planning. The Care Manager must be a highly organized professional with great attention to detail. Adaptability to frequent change is essential. Compliance with regulatory and departmental guidelines and policies is required. Essential duties include identifying cases and prioritizing daily worklists to focus on patients and new admissions. Conduct and document assessments and plans of care per departmental guidelines. Participate in established daily care management touchpoints. Communicate with relevant team members concerning patient care priorities. Attend and actively participate in meetings to discuss patients' progress. Alert the care team to any concerns that could impact patient discharge. Assist with estimating expected discharge dates. Attend weekly complex care meetings and collaborate to problem-solve complex patient issues. Proactively identify high-risk cases that require escalation. Coordinate family meetings as necessary to support progression of care. Provide education on community resources and support groups to patients and families. Identify patients with barriers to discharge and communicate needs for post-discharge services. Monitor all patients throughout the day to ensure appropriate progression of care. Verify patients’ understanding and agreement of discharge plans. Consult with social workers and utilization managers as per departmental protocols.
