This position will work with the outpatient Palliative Care team to provide palliative services to patients and families throughout UCSD. The Per Diem Nurse Care Manager (NCM) will support the interdisciplinary palliative care team by providing clinical care coordination, patient triage, symptom management support, and care transitions for a medically complex patient population. Responsibilities include managing patient communication (phone/MyChart) and triaging urgent clinical needs, coordinating care across inpatient, outpatient, and community settings, supporting symptom management, medication reconciliation, and care planning, facilitating referrals (hospice, home health, specialty services) and partnering closely with physicians/APPs to ensure continuity and quality of care. This position requires strong clinical knowledge of symptom management, including pain management, and experience working with multidisciplinary teams to optimize patient care. Strong clinical, communication, and advocacy skills are essential.
Autonomous nursing care is the ability of a nurse to assess and provide nursing actions as appropriate for patient care populations based on competence, professional expertise, and knowledge. The nurse is expected to practice autonomously consistent with ANA Standards of Practice and Professional Performance. Independent judgment is expected to be exercised within the context of interdisciplinary and multidisciplinary approaches to patient care.
While not required, a cover letter is highly recommended when applying to this position. MINIMUM QUALIFICATIONS
- Must be a graduate from an accredited school of nursing.
- RN license issued by the State of California.
- BART or BLS at time of hire with commitment to get BART w/in 6 months of hire date.
- Must have a minimum of (1) one year of outpatient RN experience working with patients at the end of life or with life-limiting progressive illnesses.
- Recent experience with processing prior authorizations.
- Proven ability to collaborate effectively with interdisciplinary teams to manage a caseload effectively, including intake and triage of symptom calls and coordination of care.
- Demonstrates excellent verbal and written communication skills, with the ability to convey complex medical information clearly and compassionately to patients and families.
- Ability to demonstrate individualized care to vulnerable populations.
- BSN or MSN.
- Experience with Hospice/Palliative Care.
- Bilingual: English/Spanish.
- Must be able to work various hours and locations based on business needs.
- Employment is subject to a criminal background check and pre-employment physical.
Annual Full Pay Range: $190,342 - $190,342 (will be prorated if the appointment percentage is less than 100%)
