Job Title Registered Nurse (RN) Case Manager Job Location Kansas City, MO, Independence, North Kansas City Company Specialized Home Care Job Summary We are seeking compassionate Registered Nurses (RNs) to join our team as Case Managers, working one-on-one with patients in their home environment. As a Case Manager, you will establish and oversee all aspects of patient care, creating a Plan of Care and following physician orders to achieve outcomes of increased patient independence and satisfaction.
