In-Home Prevention Family Case Manager
Together for Youth
Together for Youth seeks a Prevention Family Specialist to provide supportive in-home casework counseling, empower parents with tools, and advocate for children's safety, well-being, and permanency. You will assess families, develop treatment and discharge plans, coordinate services, monitor progress, and maintain confidential documentation while engaging schools and community agencies. A valid driver's license and ability to travel in Madison County are required. #J-18808-Ljbffr Together for Youth
