We are looking for an experienced MDS Coordinator to join our team in ensuring the highest quality care for our residents.

The MDS Coordinator provides management and leadership to direct care givers under the direction of the Director of Nursing (DON) to ensure that quality nursing care is provided to each resident in accordance with current federal, state, and local regulations. Oversee and coordinate the federally mandated Resident Assessment Instrument (RAI) assessment and

care planning process.

DUTIES AND RESPONSIBILITIES:

· Understand and interpret all regulations and standards related to the RAI process (Federal regulations

and MDS User's Manual).

· Assure that all components of the RAI process are completed in accordance with federal and state

regulations and facility policy and procedure. This responsibility includes, but is not limited to:

· Schedule all MDS assessments: annual, quarterly, admission, change in condition, according to

mandated guidelines

· Assures base line care plans are completed and given to patients within 72 hours of admission.

· Over-see and assist with the completion of the MDS assessments working closely with charge nurses on the skilled divisions. Check all assessments for accuracy and completeness within timeframes defined by regulatory and PPS requirements.

· Work closely with the social worker to coordinate scheduling of care plan meetings within mandated

guidelines.

· Coordinate development of each skilled resident’s interdisciplinary plan of care through the use of the

Care Area Assessments (CAA).

· Ensure that appropriate modification to the resident’s plan of care is done following incidents, accidents,

identification of risk factors (i.e. falls, weight loss, skin breakdown, wandering, infection, elopement).

· Provide in-service training for nursing staff regarding the MDS and care planning process a minimum of

annually. Assist the DON in additional in-service training as indicated.

· Maintain proficiency in the operations of the MDS software program including the ability to transmit data

per regulatory standards

· Identify residents at risk (i.e. falls, weight loss, skin breakdown, wandering, infection, elopement) and

work in collaboration with the Resident at Risk (RAR) Team to define and implement plans to reduce risk,

as directed by the DON.

· Participate in the monitoring and tracking of outcomes related to quality care as assigned by the DON.

This may include, but is not limited to, tracking of weight changes, falls, incidents, medication errors and

antibiotic use.

· Participates in the QUAPI Program. Monitors Quality Indicators, Quality Measures

and reports to committee as scheduled

· Indirectly supervises all employees within all department(s) as it relates to the roles and responsibilities of

the MDS Coordinator

QUALIFICATIONS:

· Must be a Registered Nurse currently licensed in the state of Missouri.

· Must be certified as an Infection Preventionist through CDC. The Infection Preventionist must have initial

training in either nursing, medical technology, microbiology, or epidemiology and may possess

additional training in infection control.

· Must have a working knowledge of long-term care operational standards.

· Must have knowledge of the rules and regulations inherent in completion of the MDS.

· Must have the ability to be pleasant, cheerful, tactful and courteous when dealing with residents,

employees and families.
