Rather than caring for several patients simultaneously, home-based nursing allows you to focus on the patient and family in front of you.Your visits may include:Nursing assessments and monitoring for changes in conditionWound and ostomy careMedication education and managementInjections and IV therapyPatient and caregiver educationChronic and complex disease managementComfort and symptom management for Hospice patientsCommunication with physicians and other members of the care teamCoordination with nurses, therapists, social workers and other disciplinesDocumentation in EpicWhat you'll needCurrent Wisconsin RN licenseCurrent BLS certificationValid driver's license and acceptable driving recordStrong assessment, critical-thinking and communication skillsAbility to independently prioritize patient needs and manage a field-based scheduleExcellent time management skillsApproximately two years of RN experience preferredHome Health experience preferredExperience in acute care, medical-surgical nursing, wound or ostomy care, IV therapy, infusion, oncology, rehabilitation or other hands-on clinical settings may translate well to this role.New to Home Health or Hospice?Don't rule yourself out.
