And most importantly, as the only non-profit hospice provider in the region, we never turn anyone away due to an inability to pay.The RN Case Manager is responsible for planning, managing, and coordinating care for an assigned caseload of patients receiving supportive care services. This primarily telephonic role ensures comprehensive, compassionate care through collaboration with an interdisciplinary team. Responsibilities include conducting assessments, care planning, symptom management, patient and caregiver education, and coordination with medical providers and support services.Position Details:Days/Hours: Monday - Friday, 8am-5pm; no weekendsWork Locations: (50%) Home care around the greater Gainesville area, (50%) Work from Home/phone patient/family callsCompensation: $68,000 - $72,000 base salary, plus driving paid mileage; full benefits package including 23 days PTO annualJob ResponsibilitiesConduct initial and ongoing assessments of patients' physical, functional, psychosocial, and environmental needs.Coordinate and implement individualized plans of care aligned with patient and caregiver goals.Facilitate goals of care discussions, including advance care planning.Provide telephonic support, guidance, and education to patients and caregivers.Collaborate with interdisciplinary team members, including medical providers and social workers.Monitor patient conditions and communicate symptom management needs.Ensure timely and accurate documentation in accordance with organizational standards.Participate in interdisciplinary group (IDG) meetings and care planning reviews.Comply with applicable local, state, and federal regulations.Maintain professional relationships with facilities and community partners.Participate in professional development and mentor new staff as needed.Perform other related duties as assigned.Experience and Licensures RequiredMinimum of three (3) years of experience as a Registered NurseOne (1) year of hospice or palliative care experience preferredRegistered Nurse license in the state of Florida, up-to-dateCurrent CPR certificationAssociate Degree or Bachelor's Degree in Nursing from an accredited nursing programCommunity Hospice & Palliative Care, including all of its subsidiaries and affiliated companies, is committed to complying with all applicable provisions of the Americans with Disabilities Act (ADA), as amended.
