Schedule: Per diem 1 day a week

5-6 patients a week

Territory: Macedonia, Twinsburg, Parma, Cleveland and surrounding areas

As a Medical Social Worker (MSW), you will:

• Complete comprehensive patient assessments.
• Participate in developing individualized Plans of Care.
• Identify barriers to care and recommend appropriate interventions.
• Connect patients and families with valuable community resources.
• Assist with financial assistance applications and long-term planning.
• Educate and support patients and caregivers throughout their care journey.
• Provide counseling related to adjustment, coping, loss, and grief.
• Collaborate with physicians, nurses, therapists, and the interdisciplinary care team.
• Complete accurate clinical documentation in accordance with agency, Medicare, and federal guidelines.
• Observe, document, and communicate changes in patient condition.
• Attend required training and agency meetings.

Qualifications

• Current Medical Social Worker (MSW) license in the state of employment (limited licenses not accepted)
• Master's Degree in Social Work (MSW) required
• Minimum of one year of MSW experience in an acute care setting
• Home health experience preferred
• Valid driver's license and current auto insurance
• Ability to meet the physical demands of the position
