Home Health Nurse- LPN/ RN

Position Overview We are seeking a compassionate, highly skilled nurse to provide one‑on‑one care for an 85‑year‑old female patient with complex medical needs in a warm, family‑oriented, smoke‑free, pet‑free home. The patient is a beloved matriarch of a large, supportive family and requires dedicated nursing care for multi‑system health challenges. A caregiver is also present for the patient’s husband, a Korean War veteran. Clinical Details Patient: 85‑year‑old female Care Needs: Trach care, indwelling catheter management, and total care (non‑weight bearing) Medical Profile: History of stroke/TIAs, type II diabetes (insulin‑dependent), bilateral foot drop, diabetic neuropathy, chronic UTIs, oxygen PRN Environment: High‑support home with an involved family; patient is hard of hearing (uses hearing aids) Available Shifts Days: 8:00 AM – 8:00 PM (Thursdays) Nights: 8:00 PM – 8:00 AM (Wednesdays, Thursdays, and every other Friday/Saturday) Responsibilities Provide skilled nursing care (trach/vent care, medication administration, catheter care) Monitor and document patient condition and vital signs Assist with activities of daily living (ADLs) Communicate with families and care teams Maintain safe, high‑quality, patient‑centered care Schedule & Pay Full‑time and part‑time shifts available Day, night, and weekend shifts Weekly pay with optional daily pay Competitive pay based on experience Benefits One‑on‑one patient care Flexible scheduling Paid training and shadow shifts 24/7 clinical support Career advancement opportunities Health benefits and sick time for eligible employees Qualifications Active RN or LPN license in the state of Michigan Home care or private duty experience preferred (not required) Trach/vent experience a plus (training provided) Strong communication and critical thinking skills Patient Population Pediatrics (birth–21 years) Adults with complex medical needs About Centria Healthcare Centria Healthcare is a Michigan‑based home healthcare provider specializing in private duty nursing, complex pediatric care, and catastrophic injury services.
