We are seeking a compassionate and skilled  Licensed Practical Nurse (LPN)to provide high-quality nursing care to residents in accordance with physician orders and within the scope of licensure.

Key Responsibilities:

• Administer medications and perform prescribed treatments, including wound care, tracheostomy,  gastrostomy/jejunostomy, and catheter care.
• Monitor and report residents’ responses to medications and treatments.
• Assess residents’ needs and communicate effectively with residents, families, and the interdisciplinary care team.
• Complete admission assessments and assist with MDS assessments, care plans, and quarterly reviews.
• Maintain accurate clinical and administrative documentation.
• Participate in shift reports, staff meetings, and the Quality Assurance and Performance Improvement (QAPI) program.
• Ensure medications and clinical supplies are adequately stocked and available.
• Recognize and respond appropriately to medical emergencies and changes in residents’ condition.

Qualifications & Skills:

• Current LPN/LVN license in the applicable state.
• Knowledge of geriatric nursing, pharmacology, treatment procedures, and applicable nursing regulations.
• Strong observation, assessment, communication, and interpersonal skills.
• Ability to work effectively as part of a team, exercise sound judgment, and maintain professional relationships.
• Commitment to providing safe, compassionate, and resident-centered care.

Education:

• High School Diploma
