Lead, Pharmacist - Clinical Care Coordinator

FTE = 0.625 Lead Assignment Performs all essential duties of primary position/functional area of focus. Implements practices that ensure clinical standards are met. Provides support and guidance to employees within the same functional area. Provides input for the performance assessment of other employees within the same job classification. Conducts research to identify areas for improvement. Evaluates clinical outcomes to identify areas for improvement. Keeps up to date with the latest developments in the field. Assists with ordering supplies for the department. Ensures that the department has timely access to supplies and logistical support. May assist with creating and maintaining the departmental schedule. Ensures staff compliance with clinical and operational protocols, policies, and procedures. Assists with the orientation and training of new staff. Assists with the direction and guidance for departmental workflow. Assists with daily distribution of the workload. Pharmacist - Clinical Care Coordinator duties Order Set Review Policy and Procedure updates and implementation Medication library maintenance in EHR, IV pumps, a syringe pumps Update Pharmacy reference documents Develop and update training materials for new employees Liaison for IT pharmacist or requests that affect Pharmacy department Coordinate monthly sampling for sterile prep areas Member of interdisciplinary committees including but not limited to: Pharmacy and Therapeutics, Code Blue Primary preceptor for new pharmacist, pharmacist students, pharmacy residents, and pharmacy technician enrolled in the hospital pharmacy technician training program Qualifications / Experience Require two years recent hospital experience. Prefer Hospital Pharmacy Residency Prefer Doctorate of Pharmacy degree Essential Duties A licensed Pharmacist is responsible for insuring that medications and IV's are prepared, dispensed, and delivered to nursing service in an accurate and timely manner. The Pharmacist receives orders written, phoned, or faxed by credentialed medical staff and enters them into the patient profile via the computer. They prepare or supervise the preparation of all IV's and IV admixtures by a Pharmacy Technician, Level A. It is the Pharmacist's responsibility to check all work done by a Level A Pharmacy Technician before it is delivered to a nursing area. To maintain an appropriate level of stock, the Pharmacist places medication orders with our designated wholesaler and/or manufacturers. Education is provided to patients, nurses, medical staff, and any other group that requests information on medication. Upon request of and in the absence of the Pharmacy Manager, the Pharmacist may supervise the other Pharmacy personnel. Principle Functions Dispenses or supervises the dispensing of all medication throughout the hospital utilizing a modified unit dose system. Maintains a working inventory of all medications. Places orders with wholesaler and/or manufacturers as needed. Receives and interprets physician orders then enters orders into patient profile in computer noting patient allergies. Clinically addresses each order for age-specific dosing, checks for drug interactions, and if appropriate, assures via Lab tests (MIC's) that the indicated antibiotic is ordered. When necessary, confers with physician re: changes in medication, dosage, schedule, etc. Records and investigate Adverse Drug Reaction Reports. Maintains an accurate, current inventory of narcotics in WHIDBEYHEALTH. Delivers or oversees the delivery of narcotics to the various nursing stations. Orders narcotics/controlled substances as needed. Verifies all work of Level A Pharmacy Assistants and Pharmacy Interns/Externs. Participates in education of patients, nursing staff, medical staff and the public when appropriate. Researches and responds in a timely fashion to all questions from health care workers. Performs additional duties as assigned by Pharmacy Manager. Interacts with patients and families, nurses, physicians, and other hospital staff and detailers from various manufacturers. Oncology Pharmacists, who obtain a Board Pharmacy Oncology certification, will primarily focus in Hematology/Oncology and will provide advanced clinical pharmacy services. Oversees the application of pharmacy law with pharmacy/infusion policies and procedures. Job Knowledge & Qualifications Education Bachelor of Pharmacy or PharmD from an accredited college of pharmacy. Doctor of Pharmacy degree preferred. Training and Experience Prefer two years recent hospital experience. Must communicate effectively in both oral and written format. Must organize and plan work efficiently, demonstrate good judgment and supervisory skills. Must work well with other people in a "team" environment. Certificates, Licenses, Registrations Current Pharmacist License from the Washington State Department of Health. Board of Pharmacy. Current BLS required.
