As a leading provider of remote patient monitoring, chronic care management, and collaborative care management, we're seeking a dedicated and compassionate Nurse Practitioner (NP) to join our team.
About the Role
As a Nurse Practitioner , you will play a vital role in delivering high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. You will build strong relationships with the facility, maintain a patient-centered approach, and collaborate with our clinical team to provide exceptional care.
Responsibilities
Provide preventative care to residents in skilled nursing facilities
Conduct on-site visits and remote monitoring to identify potential health issues
Develop and implement personalized care plans to minimize hospitalizations
Collaborate with our clinical team to ensure seamless care coordination
*-document patient information and treatment plans efficiently and accurately
Requirements
Active Nurse Practitioner (NP) license in the state of Illinois
1+ year of experience in Geriatric care and documentation review
Excellent communication, collaboration, and teamwork skills
Strong patient-centered approach and commitment to preventative care
