Description

• Assess patients in both inpatient and outpatient modalities and develop tailored rehabilitation plans
• Implement interventions that enhance mobility, strength, balance, and overall functional capability
• Work in tandem with physicians, nurses, case managers, and rehabilitation specialists to ensure optimal patient outcomes
• Guide patients and families through educational sessions regarding treatment objectives, discharge criteria, and at-home practices
• Maintain accurate documentation of evaluations, treatments, and progress as per healthcare regulations and standards
• Engage actively in team meetings and interdisciplinary discussions for seamless patient care coordination

Education

• Doctorate in Physical Therapy (DPT) or equivalent qualification from an accredited institution

Certifications

• Current state licensure as a Physical Therapist
• CPR certification is preferred
• Additional certifications in specialized areas (neurological, geriatric) are advantageous but not mandatory

• Previous clinical experience is highly desirable
