RN Care Manager - Senior Home & Clinic Nurse Leader

BoldAge PACE in Columbus, OH is seeking a Registered Nurse Care Manager to assess care needs, provide nursing interventions, and evaluate outcomes for participants in clinic, center and home settings. You will work with the interdisciplinary team to develop plans of care and delegate tasks to aides as needed. The role requires RN license, BSN preferred, and BLS; prior home or managed care experience is valued.
