Job Title: RN Case Manager (Inpatient) Location: 500 Upper Chesapeake Dr, Bel Air, MD 21014 Duration: 13 Weeks | Start: 05/04/2026 or ASAP Shift Details Day Shift: Monday-Friday
08:00 AM - 04:30 PM
Job Summary Seeking an experienced RN Case Manager (or SW preferred) with inpatient case management experience to handle complex discharge planning, legal coordination, and patient support services. Responsibilities Manage ~15 inpatient cases daily Coordinate safe discharge planning, placement, and home care services Handle complex cases including locating family/support systems Work on guardianships, capacity evaluations, and certificates of competency Collaborate with legal teams and attend court hearings with patients Address social, medical, and placement-related needs Requirements RN with Inpatient Case Management experience (SW preferred) Strong experience in discharge planning and care coordination Ability to manage complex psychosocial and legal cases Patient Focus Inpatient population requiring medical, social, and placement support services Additional Details Holidays: Required as scheduled #J-18808-Ljbffr Chapter 13 Trustee Middle District of AlabamaVacancy posted 3 days ago
