Senior Care Management Social Worker: Link & Coordinate Care

Blue Shield of CA is seeking an experienced Care Management Clinical Social Worker to join the team supporting members with complex social determinants of health. You will assess psychosocial needs, develop care plans, and provide intensive interventions in collaboration with nurses and providers. Reporting to the Care Management Manager/Senior Manager, you will link members to appropriate care levels and community resources, coordinating across care teams to improve outcomes and access to #J-18808-Ljbffr Blue Shield of CA
