Responsibilities Conduct assessments to determine clients’ social, emotional, and psychological needs. Develop and implement personalized care plans in collaboration with clients and their families. Provide counseling and support to clients and their families to navigate challenges. Coordinate with healthcare providers to ensure holistic care delivery. Facilitate support groups and educational workshops for clients and caregivers. Maintain accurate and confidential client records in compliance with regulations. Advocate for clients’ rights and access to community resources. Stay updated on best practices and trends in social work and elder care. Requirements Master's degree in Social Work (MSW) or other related field or Bachelors degree in Social Work or other related field and 2 years of experience in a healthcare or social services setting Strong communication and interpersonal skills with a compassionate demeanor. Ability to work collaboratively in a multidisciplinary team environment. Knowledge of community resources and services for seniors. Proficient in documentation and familiarity with electronic health records. Commitment to ongoing professional development and ethical practice.
