Role Overview The Medical Social Worker applies social work expertise and clinical knowledge to educate patients living with kidney disease, supporting their social and emotional well-being. They conduct psychosocial assessments, participate in care planning, and manage services to help patients cope with chronic kidney disease and dialysis treatments. The Medical Social Worker applies social work expertise and clinical knowledge to educate patients living with kidney disease, supporting their social and emotional well-being. They conduct psychosocial assessments, participate in care planning, and manage services to help patients cope with chronic kidney disease and dialysis treatments. Key Responsibilities Support patients’ social and emotional needs through assessments and interventions. Collaborate with a multidisciplinary team to develop personalized care plans. Educate patients on treatment options, health, and wellness to improve quality of life. Assist patients in managing issues related to kidney disease and dialysis. Qualifications & Skills Master’s Degree in Social Work from an accredited CSWE institution. Required licensure: MSW; LCSW/ACSW preferred (CA), LMSW in states with licensure (TX, TN). Preferable experience in healthcare or behavioral health settings; medical experience is highly desirable. High energy, goal-oriented, with strong relationship-building skills and a sense of purpose.
