Key Responsibilities

• Conduct detailed patient evaluations through comprehensive history reviews and targeted assessments, to develop individualized treatment plans based on specific needs.
• Engage with patients and their families to clearly communicate evaluation findings, treatment strategies, and to collaboratively set achievable goals.
• Utilize therapy protocols effectively, tailoring interventions according to patient presentations and evolving healthcare guidelines.
• Collaborate with other healthcare professionals, ensuring necessary referrals and consultations are made to modify treatment plans when a patient's condition changes.
• Oversee discharge planning and care continuity; meticulously document patient progress and treatment interactions to meet both regulatory requirements and clinical standards.
• Exercise strong time management skills to meet productivity benchmarks while maintaining an accurate patient schedule.
• Participate actively in departmental meetings and contribute to the achievement of departmental objectives and compliance with healthcare regulations.
• Pursue continuing education opportunities to enhance professional skill sets and mentor students as a clinical instructor.
• Advocate for the best patient experience, addressing barriers to access and engaging in departmental initiatives to improve care delivery.

Patient Population / Client Focus

Our clinic predominantly serves individuals recovering from orthopedic surgeries, neurological conditions, sports injuries, and age-related physical limitations. We cater to a diverse patient demographic, including seniors, athletes, and individuals requiring rehabilitation following accidents or surgeries. Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• Current and valid state licensure to practice as a Physical Therapist in Connecticut.
• Certification in Basic Life Support (BLS) from the American Heart Association is mandatory.

• A minimum of 2 years of clinical experience in a rehabilitation or outpatient therapy setting is preferred but not required.

Compensation and Benefits
