Oasis Therapy Services is seeking a Physical Therapist (PT) to provide one-on-one rehabilitation services in an assisted living setting in Oshkosh, Wisconsin.

Available employment options:

• Part-Time or PRN

EMR: Net Health

Responsibilities

• Evaluate mobility, strength, balance, gait, and fall risk.
• Develop and provide individualized treatment plans to improve safe mobility and independence.
• Monitor progress and adjust treatment plans as needed.
• Educate residents and caregivers on exercises, transfers, fall prevention, and assistive devices.
• Complete accurate Net Health documentation and collaborate with the care team.

Qualifications

• Active and current Wisconsin Physical Therapist license.
• Graduate of an accredited Physical Therapy program.
• Current CPR certification.
• Strong communication, organization, and documentation skills.
• Geriatric, assisted living, skilled nursing, or rehabilitation experience is preferred.
