Consonus Pharmacy is a closed-door long-term care pharmacy specializing in the needs of nursing facilities, assisted living residents, and their caregivers. We are currently hiring a Consultant Pharmacist to join our team that lives in the Eugene or Medford area! As the Consultant Pharmacist your responsibilities will include, but are not limited to:Review resident medication regimens monthly (or as agreed upon) per patient acuity level and report findings in writing to facility administrator, director of nursing and physicianPlan and present in-service education to other pharmacy staff and nursing facility employees as requestedUnderstand and implement all appropriate pharmacy policies & proceduresParticipate in the development of emergency drug supply kits at each facility in conjunction with the medical director, provider pharmacy and nursing administrationLiaison between facility and pharmacy staffProvide ongoing education directly or indirectly to residents preparing for discharge from facilities servicedKnowledge of CMS regulations and interpretive guidelinesAttend quality assurance and other appropriate committee meetings as requested by facilitiesPrepare quarterly quality assurance reportsBenefits may include:Paid training & career advancement opportunities$25,000 toward ongoing education benefitGenerous Paid Time Off that pools vacation, sick & holiday pay in a single bucket for flexible useMedical/Dental/Vision insuranceFree access to MDLIVE – allowing access to your health benefits from anywhere401K With Employer MatchEmployee Assistance Program: with free access to counseling & support servicesQualifications:B.S. Pharm.
