Description

• Evaluate and assess patients in both inpatient and outpatient settings, creating personalized treatment plans that cater to their individual needs.
• Implement therapeutic interventions focused on enhancing mobility, strength, balance, and overall function.
• Collaborate with a diverse team of nurses, physicians, case managers, and other rehabilitation experts to maximize patient outcomes.
• Educate patients and their families about therapeutic objectives, discharge planning, and at-home exercises.
• Document patient evaluations, treatments, and progress, adhering to facility and regulatory standards.
• Engage in interdisciplinary care team meetings and rounds to promote coordinated patient care.

Education

• Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution.

Certifications

• Valid state licensure as a Physical Therapist .
• Preferred CPR certification.
