Housing Specialist

LaSante Health Center is seeking a compassionate and organized Housing Specialist to support patients experiencing housing instability, eviction risk, homelessness, rental arrears, and other housing‑related challenges. Responsibilities: Assess patients’ housing needs and develop action plans. Assist with affordable housing, supportive housing, NYCHA, Section 8, shelter, rental‑assistance, and eviction‑prevention applications. Help patients gather documents and complete applications. Coordinate with landlords, housing agencies, legal providers, shelters, and community organizations. Document all patient interactions, referrals, outreach, and follow‑up activities accurately and promptly. Track applications, deadlines, and patient progress. Work closely with therapists, case managers, and medical providers. Qualifications: Experience in housing services, case management, social services, or a related field preferred. Familiarity with New York City housing resources required. Strong communication, organization, documentation, and follow‑up skills. Ability to work compassionately with vulnerable individuals and families. High school diploma or equivalent required. Bilingual skills are a plus. #J-18808-Ljbffr Lasante-Health-Center
