Field Nurse Case Manager - St. Louis, MO St. Louis, MO, USA Job Description Paradigm is a specialty care management organization focused on improving the lives of people with complex injuries and diagnoses. Paradigm is looking for a full‑time, 0.75‑1.0 FTE (30‑40 hours per week) Field Nurse Case Manager to cover the St. Louis, MO area. This is a salaried position: base salary range at 0.75 FTE is $64,350‑$70,200; at 1.0 FTE it is $81,000‑$93,600 annually. The Field Case Manager is a home‑based position with up to 2 hours one‑way travel (4 hours round‑trip) to doctors’ offices, hospitals, and other locations. The individual is responsible for the medical case management of work‑related injuries, which includes assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager works with insurance carriers, medical care providers, attorneys, employers, and employees, and closely monitors the progress of the injured worker and reports results back to the employer and insurance carriers. Qualifications Current, unencumbered compact Registered Nurse (RN) license in Missouri required. Experience in case management of workers’ compensation cases strongly preferred. Recommended certifications (CCM, CRRN, CDMS, CRC) strongly preferred (must obtain within two years of hire date). Skills required for success: organization and timeline adherence, PC and technology proficiency, and written, verbal, and interpersonal communication skills. Ability to operate autonomously with minimal oversight. Skilled at patient education. Valid driver’s license and good driving history.
