Responsibilities
- Evaluate and treat residents requiring physical therapy within geriatric rehab.
- Develop and implement individualized plans of care based on evaluation findings.
- Administer therapeutic exercises, gait training, manual therapy, modalities, and functional activities.
- Reassess progress regularly and adjust the plan of care to achieve goals.
- Document assessments, treatments, and progress accurately in the resident’s records.
- Collaborate with an interdisciplinary team including therapy, nursing, and medical staff to coordinate care and discharge planning.
- Ensure compliance with regulatory standards, patient privacy (HIPAA), and facility policies.

Requirements
- Active Physical Therapist license in Maryland or eligibility for licensure in Maryland
- Doctor of Physical Therapy (DPT) degree or equivalent
- Strong clinical skills in geriatric rehab and skilled nursing facility settings
- Excellent communication and documentation abilities
- Ability to work 35 hours per week in a 5x8 day schedule
- Willingness to travel for a temporary assignment
