Case Manager Long Beach, CA Salary: $23.00 to $28.00 per hour Pay Frequency: 24 Pay Periods Per Year Full Time Monday through Friday 8:30 am to 5:00 pm Non-Exempt Driving Required PATH is seeking candidates passionate about helping others make a positive change in their lives to join our Interim Housing team as the Case Manager at the Colonial site. About PATH Since its foundation in 1984, PATH (People Assisting The Homeless) has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting clients to a continuum of homelessness prevention, street outreach, employment preparation and placement, individualized case management, supportive services, interim housing, and permanent supportive housing. About The Job The Case Manager role’s main function is to assist clients in breaking the cycle of homelessness by utilizing a “whatever it takes” approach. This position supports clients through all phases of homelessness, including on the street, interim housing, transitional housing, and permanent housing. They assist clients with accessing and maintaining necessary services among healthcare, social services, interpersonal support systems, and navigating a coordinated entry system (CES). Position Responsibilities Conduct intake and individualized needs assessment for all participants and collaborate to develop an Individualized Service Plan (ISP) addressing housing goals, income, and other personal goals (medical, mental health, substance use, financial resources, vocational, social support). Provide referrals, linkages, information, and support to resources that help participants achieve their ISP goals. Complete program-specific assessments (e.g. VI-SPDAT). Evaluate strengths and challenges to address short- and long-term goals; conduct a 90-day review with the participant after intake. Maintain documentation standards as required by the Program and PATH policies, including timely case notes and an accurate participant file with all required documents. Maintain confidentiality of participant files per HIPAA and applicable guidelines. Develop therapeutic and professional relationships with clients and partners to engage and retain individuals with housing goals. Provide crisis intervention and risk assessment services focused on safety and helping clients independently solve problems and manage their own care, including services for individuals with chronic trauma. Use motivational interviewing and trauma-informed care in service delivery. Maintain current knowledge of community resources and utilize them to provide comprehensive wrap-around services to clients. What You Bring Analytical Skills and Attention to Detail: Gather and interpret data, identify trends, and inform decision-making with high-quality, accurate work. Collaboration and Teamwork: Build positive working relationships with colleagues, participants, and stakeholders; contribute to team goals. Communication: Convey information clearly and effectively in writing and speech. Creativity and Initiative: Take action, think innovatively, and take on additional responsibilities when needed. Critical Thinking and Problem Solving: Analyze information, identify issues, consider alternatives, and make logical decisions. Ethics and Boundaries: Maintain ethical standards and professional boundaries. Preferred Qualifications Two (2) years of experience in healthcare, case management, homeless services, or working with vulnerable populations or related field. Minimum Qualifications All levels of education and experience are welcome. Mandatory Requirements Regardless of qualifications, candidates must: Have employment eligibility verification Have or be able and willing to obtain CPR/First Aid training Successfully complete the following as a condition of hire: Tuberculosis Test; Background Screening; Drug Test Driving Driving is an essential function of this position.
