TLS Case Manager - RHC (5657)

Overview PATH is seeking candidates passionate about helping others make a positive change in their lives to join our Time Limited Subsidy team as the Case Manager at the RHC location in Los Angeles, CA. About PATH Since its foundation in 1984, PATH (People Assisting The Homeless) has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing. About Our Team Every program in our growing range of services is designed to holistically address barriers to long-term housing stability, implementing evidenced-based practices to promote well-being beyond housing. About The Job As part of the Time Limited Subsidy team, the Case Manager’s main function is to assist clients in breaking the cycle of homelessness by utilizing a “whatever it takes” approach. This position supports clients through all phases of homelessness, including street outreach, interim housing, transitional housing, and permanent housing. They assist clients with accessing and maintaining necessary services among healthcare, social services, interpersonal support systems, and navigating through a coordinated entry system (CES). The position will provide individualized supportive services by helping each client develop a plan to address strengths, barriers, health needs, income, and housing goals. Position Responsibilities Conduct intake and individualized needs assessments for all participants and collaborate to develop an Individualized Service Plan (ISP) that addresses housing goals, income, and other personal goals (medical, mental health, substance use, financial resources, vocational, and social support needs). Provide referrals, linkages, information, and support to resources that help participants achieve ISP goals. Complete program-specific assessments (e.g., VI-SPDAT). Evaluate strengths and challenges to address short- and long-term goals; conduct a 90-day review with the participant after initial intake. Maintain documentation standards as required by the Program and PATH policies, including timely case notes and a complete, accurate, and current participant file with all required documents. Maintain confidentiality of participant files per HIPAA and applicable guidelines. Develop therapeutic and/or professional relationships with clients and systems/partners to engage and retain individuals with housing goals. Provide crisis intervention and risk assessment services focused on increasing safety and empowering clients to solve problems and manage their own care, including services for individuals with chronic trauma. Use motivational interviewing and trauma-informed care in service delivery. Maintain current knowledge of community resources and utilize them to provide comprehensive wrap-around services to clients. What You Bring Analytical Skills and Attention to Detail: Gather and interpret data to inform decision-making; produce high-quality work with accuracy. Collaboration and Teamwork: Build positive relationships with colleagues, participants, and stakeholders; contribute to team goals. Communication: Convey information clearly and effectively in written and verbal form. Creativity and Initiative: Take action, proactively take on responsibilities, and generate innovative solutions. Critical Thinking and Problem Solving: Analyze information, identify key issues, consider alternatives, and make sound decisions. Ethics and Boundaries: Maintain ethical and professional standards with integrity and appropriate boundaries. Preferred Qualifications Two (2) years of experience in healthcare, case management, homeless services, or working with vulnerable populations or a related field. Minimum Qualifications All levels of education and experience are welcome. Mandatory Requirements Regardless of qualifications, candidates must: Have employment eligibility verification Have or be able and willing to obtain CPR/First Aid training Successfully complete the following as a condition of hire: Tuberculosis Test Background Screening Drug Test Driving Driving is an essential function of this position. To meet employment requirements, all candidates must: Have reliable transportation A valid driver’s license Proof of insurance and ownership for personal vehicles used during work duties The ability to qualify for PATH’s insurance coverage Employment Eligibility Applicants must be authorized to work for any employer in the U.S. We are unable to sponsor or take over sponsorship of an employment visa. California-Based Work Expectations We are a California employer. All regular work must be performed within the state. Approval for work outside of the office does not establish approval for work outside of California. PATH does not offer fully remote positions. Some roles may include a schedule with multiple worksites or approved off-site locations, but many positions are fully on-site. Work schedules are determined by responsibilities and may be adjusted at any time.
