Nurse Practitioner or Physician Assistant

Non-profit integrated community health center has need for part-time or full time physician assistant or nurse practitioner to add to our Primary Care Team. We are an integrated health care facility and as such offer primary care, pediatrics, same day services, and behavioral health services, including psychiatry, counseling, and a substance use disorder clinic. We offer on-site digital x-ray, in-house CLIA-waived testing, perform EKGs, PFTs, injections and are a Vaccines for Children clinic in addition to being recognized by NCQA for our Patient Centered Medical Home, as well as a Tier Five Primary Care Medical Home as designated by the Oregon Health Authority. We are also a preceptor site for multiple physician assistant and NP programs and are dedicated to teaching the next generation.

*Responsibilities*

* Conduct patient assessments and develop individualized care plans
* Utilize EHR systems to document patient information and treatment plans
* Collaborate with healthcare teams as needed to ensure quality patient care
* Educate patients and families on health management and treatment options

We are looking for long-term care providers who wish to make a difference in their patients' lives. Work as much or as little as you wish, 24-32 hours per week depending on your needs, there is no holiday work and limited call.
