In Home Counselor

Location: Hybrid

Type: Full-time or Part-time

Program: Mobile Crisis Response, Community Stabization, and/or Mental Health Skill Building

Population: Adults (18 and older)

Job Overview:

We are seeking a dedicated In Home Counselor to join our team. As an In Home Counselor, you will provide therapeutic intervention, skill restoration, and psychoeducation to adult clients. This is a hybrid (remote and in client’s home) hourly, PRN position, where you make your own schedule with clients.

Duties:

Provide therapeutic services based on the client’s treatment plan.
Provide education to clients and their families regarding mental health conditions, treatment options, and preventive measures
Collaborate with other healthcare professionals to ensure coordinated and comprehensive client care
Document client progress on weekly case notes and quarterly reports.
Monitor client progress and make necessary adjustments to treatment plans
Participate in weekly team meetings and treatment planning.

Requirements:

Bachelor degree in Psychology, Social Work, Counseling, Sociology, and any other related field
