Physical Therapist (Outpatient PT)

Location: Santa Rosa, California
Contract Length: 13 Weeks
Shift: Day Shift | 8:30 AM – 5:00 PM
Schedule: 5x8 Hour Shifts

Job Summary

"Navitas Healthcare, LLC" is seeking an experienced Physical Therapist for an outpatient assignment within an acute care hospital setting. The ideal candidate will have strong outpatient rehabilitation experience and willingness to float to acute rehab with proper orientation. The PT will evaluate patients, develop individualized treatment plans, and provide high-quality rehabilitative care to improve mobility and functional outcomes.

Required Qualifications
• Minimum 3 years of Physical Therapy experience required
• Active California PT License required
• Acute rehab experience preferred or willingness to float with orientation
• Strong outpatient rehabilitation experience required
• EPIC charting experience preferred
• Pending license not accepted

Key Responsibilities
• Evaluate patients and develop individualized treatment plans
• Provide therapeutic exercises and rehabilitation treatments
• Monitor patient progress and adjust care plans as needed
• Educate patients and families regarding recovery and mobility goals
• Maintain accurate documentation in EMR systems
• Collaborate with physicians and interdisciplinary healthcare teams
• Float between like units as needed
• Follow hospital safety, infection control, and therapy protocols
