Position: Licensed Practical / Vocational Nurse – LPN/LVN Responsibilities: Provide high-quality home healthcare to adult patients with complex needs, including trach/vent and high acuity cases. Ensure clinical outcomes through compassionate care, documentation via electronic tablets, and collaboration with a multidisciplinary team. Support patients' well-being and safety, adhere to care plans, and maintain compliance with health standards.
