Role Overview As a Hospice RN, you will serve as more than just a nurse; you will be a clinical leader, advocate, and a steady source of support for patients and their families. Your responsibilities will include leading care plans, managing symptoms, and ensuring that each patient receives personalized, comfort-oriented care that aligns with their goals and preferences. Your Responsibilities Lead Patient-Centered Hospice Care: Conduct thorough assessments of patients and their families. Create and oversee individualized care plans focused on comfort. Administer skilled nursing care with an emphasis on pain and symptom management. Serve as the Care Coordinator: Act as a case manager, ensuring coordinated care across the interdisciplinary team. Collaborate with physicians, social workers, chaplains, and aides. Monitor patient conditions and adjust care plans to ensure comfort. Support Patients & Families: Provide education, emotional support, and guidance throughout the care journey. Assist families in navigating complex decisions with empathy and clarity. Establish meaningful, trusting relationships during deeply personal moments. Drive Quality & Compliance: Maintain accurate and timely documentation. Participate in interdisciplinary team meetings and care planning. Support adherence to quality and compliance standards. Qualifications Current RN license (or eligibility for compact license). Graduate from an accredited nursing program. At least one year of clinical nursing experience. Strong clinical judgment and a compassionate approach to care. Ability to manage a caseload independently. Excellent communication skills, especially in sensitive situations. Basic proficiency in electronic medical records (EMR). Current CPR certification.
