Licensed Practical Nurse LPNJob SummaryLicensed Practical Nurse Seeking a Licensed Practical Nurse/LPN who will be responsible for providing direct patient care . Will be responsible for working collaboratively with the members of the healthcare team to help meet positive patient care outcomes.Job Type:Responsibilities and DutiesResponsibilities of Licensed Practical Nurse/LPN include but not limited to the following:Implement plan of care initiated by the clinicianProvides accurate and timely documentationAssess and provides patient and family caregiver education and information pertinent to diagnosis and plan of careParticipates in coordination of home health services to the clinician and/or clinical supervisorQualifications and skills:One year med surg experienceOne year Home Health experience required.Louisiana State Board of LPN license requiredBenefits: Full time employment onlyMedical Insurance - 50% of cost paid by employerDental, Vision, Life Insurance at Group RatesVacation Paid Time off 401 K Matching Job Types: Part-time, PRN Medical specialties:GeriatricsHome HealthSchedule:flexible hoursMonday to FridayExperience:Home health: 1 year Requiredmed surg: 1 year RequiredWork Location: One location
