We are committed to delivering the highest level of skill, compassion, and respect to our community through hospice and palliative care services.Responsibilities:• Provide a nursing evaluation visit to new admissions.• Regularly re-evaluate patient needs.• Consult with the patient's practitioner.• Complete bedside documentation using the EMR.• Coordinate and manage services offered by team members.• Help ensure the final chapter of your patient's life is one of quality, compassion, and dignity.Essential Skills for Success:• Expert clinical assessment and symptom management• Compassionate, clear communication skills• Strong interdisciplinary collaboration and care coordination• Ability to work independently with sound clinical judgment• Knowledge of hospice philosophy, ethical standards, and regulatory requirements• Emotional resilience and professionalismQualifications:• Graduation from an accredited nursing school and a valid license as a Registered Nurse in the Commonwealth of Virginia.• Two years of relevant nursing experience is preferred.• Driver's license, personal automobile, and current automobile liability insurance.Typical Physical Demands/Working Conditions:• Must be prepared to work in home environments that have difficult working conditions including, but not limited to cramped space, pets, dirt, allergens smoke, limited seating• The job requires regular use of hands for handling objects, tools, or controls, and involves sitting, standing, bending, reaching, lifting, walking, and climbing steps.
