Key Responsibilities
- Assessment & Care Planning: Complete and submit accurate Minimum Data Set (MDS) assessments (scheduled and unscheduled) according to federal and state regulations.
- Reimbursement Management: Set Resource Utilization Groups (RUGs) levels to ensure maximum, accurate reimbursement.
- Interdisciplinary Team Coordination: Lead care planning meetings, collaborate with nursing, PT/OT, and dietary staff to review patient progress and update plans.
- Quality Assurance: Monitor patient care for quality indicators, ensuring compliance with state and federal regulations.
- Compliance: Conduct interviews and review medical records to ensure all documentation justifies the care provided and reimbursement level.
Required Qualifications & Skills
- License: Active Registered Nurse (RN) license, though some facilities may accept a Licensed Practical Nurse (LPN) with extensive experience.
- Experience: Previous experience in LTC/SNF, with a strong understanding of MDS 3.0 and RAI processes.
- Knowledge: Deep understanding of Medicare/Medicaid regulations, ICD-10 coding, and care planning processes.
- Skills: Strong assessment skills, attention to detail, critical thinking, and proficiency in Electronic Health Record (EHR) systems.
Common Qualifications
An MDS Coordinator usually needs a valid state nursing license and, in some cases, a bachelor's degree in nursing.

- Conducting assessments: Assessing residents for functional, nutritional, behavioral, and physical needs.
- Updating records: Maintaining records for all residents in the facility.
- Improving care: Working with other staff members to improve patient care.
- Investigating issues: Investigating complaints from family members or residents.

- Software: Experience with software for the MDS.
