- Providing assessments of client needs toward recommending treatment modalities when appropriate. \n
- Identify clients who might benefit from information pertaining to alcohol and/or substance use or abuse. \n
- Utilizing assessment tools (potentially found in CARES and elsewhere) to identify available resources and support services and make timely referrals to those which seem most appropriate based on your understanding of clients’ individual circumstances and goals. \n
- Orienting clients to their assigned transitional housing facility and the larger New York City shelter program while informing and reminding them of the rules, policies, procedures, and schedules currently in effect. \n
- Providing clients with information and guidance on how to access or apply for available resources, such as public assistance, and alcohol or substance use or abuse treatment services. \n
- Developing Independent Living Plans (“ILPs”), in coordination with other social services providers, which are designed to deliver comprehensive support services to clients. \n
- Monitoring and tracking how clients progress in achieving their stated goals. \n
- Empowering clients to actively participate in their own planning and goal setting. \n
- Scheduling and conducting meetings with clients (on a regular one-on-one basis and as otherwise needed) to touch base, assess, and address whether there are any updates to their personal circumstances and needs. \n
- Referring clients for and encouraging them to take advantage of employment and job or skills development opportunities if appropriate. \n
- Connecting clients with suitable resources to help them achieve their goals. \n
- Meeting clients to address challenges, successes, and strengthen their capacity for long-term self-sufficiency. \n

- Serving as an advocate for clients and also connecting them with outside advocacy groups when necessary. \n
- Providing clients with periodic case management and counseling, extending one (1) year beyond their final exit from the facility. \n
- Establishing and/or nurturing connections and partnerships with other programs and units, both within and outside the organization, to facilitate referrals and support services. \n
- Coordinating and/or escorting clients to housing viewings, and DHS or HRA appointments as required. \n
- Facilitating suitable long-term housing placements for clients as quickly as possible. \n
- Submitting complete case notes within 24 hours of completing any client meeting. \n
