Work Environment\n

This role is primarily remote, providing you the flexibility to conduct assessments and sessions from the comfort of your own home or a flexible workspace as needed. The community health clinic operates Monday through Friday, 8:00 AM to 4:30 PM, with additional evening or weekend hours required occasionally to meet client needs. Comprehensive communication tools and electronic health record (EHR) systems are in place to facilitate seamless interactions with clients and interdisciplinary team members.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for seniors to evaluate emotional, mental, and social needs.

• Develop and implement personalized care plans in collaboration with clients and their families, tailoring support to align with their specific goals.

Qualifications\n

• A Master's Degree in Social Work from an accredited institution.

Compensation and Benefits\n
