• Monday – Friday 8am – 5pm with guaranteed 40 hours per week
• 5 – 6 regular visits per day, 3 visits if admission day
• Majority home health with occasional hospice evaluations
• 1 year experience required, home health experience preferred
• All ages from pediatrics to adults
• Travel PT handling start of care and recertifications
• 6.5 points per day productivity standard
• OASIS training preferred for documentation
