We’re hiring Health Coaches in Columbus, Ohio At Diverge Health we are passionate about improving health access and outcomes for those most in need. We partner with primary care providers to improve engagement and continuity of care for Medicaid patients, offering independent practices with specialized resources and clinical programs to close gaps in care. As a Health Coach, you’ll partner with patients to build healthy habits, overcome barriers, and connect with the right support and resources. What You’ll Learn Training at Diverge Health is immersive, hands‑on, and designed to fully prepare you for success as a Health Coach. It’s a core part of your journey—building the skills, confidence, and support network you’ll need to thrive in this role. During your initial 12‑week training period, you’ll participate in daily live instruction, interactive learning modules, role plays, teach‑backs, and practice sessions with real patients. You’ll then move into an apprenticeship phase for approximately two additional months, working alongside experienced coaches and receiving ongoing feedback and mentorship. What To Expect Full‑time commitment: Training runs 8 hours a day, with two days per week in‑office. Hands‑on practice: Frequent assignments, role plays, and feedback to build confidence and skill. Real‑world preparation: Learn about Motivational Interviewing, care coordination, and how to help patients navigate social needs such as food, housing, and transportation. Culture of support: Dedicated trainers, clinical mentors, and peer cohorts guide you every step of the way. What You’ll Do As a Health Coach, you will be a trusted partner to patients, helping them understand their conditions, stay engaged in their care, and take practical steps toward better health. You’ll work closely with a care team of clinicians to coordinate care and support patients holistically. Key Areas You’ll Add Value Conduct one‑on‑one coaching sessions in homes or community settings, building trusted relationships with up to 20 patients each week. Educate patients about chronic conditions such as diabetes, COPD, asthma, CHF, and hypertension. Motivate and empower patients through goal setting, accountability, and culturally sensitive health coaching. Help patients engage or re‑engage with primary care, including scheduling visits and ensuring follow‑up after hospitalizations. Support care coordination by reviewing charts, confirming appointments, and helping patients prepare for visits. Guide patients in identifying and accessing community, health plan, and government resources (e.g., food assistance, housing, transportation) using resource directories and other tools. Document patient encounters and progress accurately in our internal database‑Salesforce to inform the broader care team. Collaborate with Nurse Practitioners and Social Workers to facilitate case reviews and support patients with complex needs. Communicate patient risks or complications promptly to clinical team members for follow‑up and triage. Support other tasks as needed to best meet the needs of the patient, their primary care provider, or the care team. What You’ll Bring 2+ years of experience in customer service, community work, teaching, case management, or other people‑centered roles. High School diploma or GED required (some college or healthcare experience preferred). Strong interpersonal and organizational skills; able to build rapport quickly and follow through consistently. Comfort with technology and adaptability, experience using systems like Salesforce or willingness to learn. Ability to work independently and collaboratively across a care team. Commitment to health, wellness, and supporting others in achieving their goals. Local travel on a regular basis—required (up to 70%) within the surrounding community. In‑Person Training & Work Requirements This role includes in‑person training and patient visits. Employees living more than 60 minutes from the Diverge office during typical commuting hours may be eligible for an exemption from in‑person training requirements. Office location is subject to change based on market needs. Timeline & Compensation Anticipated start date: February 23, 2026. Hourly rate: $23.00. Eligible for consideration of an annual performance‑based bonus, contingent on individual contributions and overall company goals.
