Admissions RN – Bristol Hospice Location: Waxahachie, TX 75165 Position Type: Full Time Salary: $83,200.00 per year On an Average Day You Will: Relate information to the DPCS, ED, and Community Education Director to promote hospice access. Assess and meet community educational needs regarding hospice care and services. Track community, customer, payer, and patient perceptions of Bristol Hospice as a high‑quality provider. Facilitate communication with agencies and referral sources to promote continuity of care. Maintain comprehensive knowledge of hospice educational resources and share information with organizational personnel. Facilitate the hospice referral process and serve as a resource for patient information review. Coordinate with hospice agency leadership and referral sources to promote care coordination and timely admissions. Complete hospice consults, evaluations, and admissions. Complete initial assessments of patients and families to determine hospice needs. Provide a complete physical assessment and history of current and previous illnesses of referrals. Serve as a liaison between facility, physicians, hospitals, skilled nursing facilities, and community/hospice programs. Coordinate with ED and DPCS on the planning, implementation, and evaluation of in‑service and continuing education programs. Participate in Quality Assurance Performance Improvement (QAPI) processes, as directed. Function as an RN Case Manager and maintain an assigned patient case load, as determined. Provide professional nursing care utilizing all elements of the nursing process. Use health assessment data to determine nursing diagnosis. Participate in IDT meetings and collaborate to determine patient/family plan of care. Provide patient/family education related to the plan of care. Perform all other duties as assigned. Requirements: Current RN license in Texas. Minimum one (1) year of professional nursing experience within the last three (3) years, or a bachelor’s degree in nursing from an NLSN‑accredited program. Willingness to travel up to 20% of the time, often without notice. Flexible work hours. Ability to maintain comprehensive knowledge of information systems and applicable software programs.
