Position Overview

We are seeking a dedicated healthcare professional with home health experience. The ideal candidate will possess OASIS training and the necessary certifications. This position requires a minimum of 6 to 8 skilled home visits per day, focusing on a variety of patient diagnoses including surgical recovery and chronic conditions.

Shift Details

Shift: Day 5x8-Hour (08:00 AM - 05:00 PM)

Key Responsibilities

Conduct skilled home visits for patients with various diagnoses, including but not limited to:

Medical/Surgical
Cardiac
Orthopedic (hips, knees, shoulders)
Chronic obstructive pulmonary disease (COPD)
Congestive heart failure (CHF)
Pneumonia
Cerebrovascular accident (CVA)
Fall-related injuries
Neurological conditions
Post-surgical care
Trauma recovery

Ensure patient care plans are updated and accurately documented.
Collaborate with interdisciplinary team members to provide comprehensive care.

Qualifications

Minimum of 1 year of experience in home health care.
Current certifications required: BLS.
OASIS training is preferred.
Must possess a valid driver's license and have a personal vehicle for home visits.
COVID-19 vaccination or exemption form on file, and flu shot documentation.

Work Environment

This position may involve traveling to patient homes. The candidate should be comfortable working independently and managing their schedule according to patient needs. Daily patient visit quotas will be enforced, with minimum expectations set at the discretion of management.

Additional Information

Interactions with healthcare professionals, including physical therapists and social services.
Patients include adults and geriatrics.
Daily patient caseload: 1:6-7.
