Medical Social Worker

Role Overview The Medical Social Worker applies social work expertise and clinical knowledge to educate and support patients living with kidney disease, enhancing their quality of life. They conduct psychosocial assessments, participate in care planning, and manage services to help patients cope with chronic kidney disease and dialysis treatments. Key Responsibilities Support the social and emotional well-being of patients. Collaborate with a multidisciplinary team to develop personalized care plans. Participate in team reviews of patients' psychosocial needs. Assist patients in managing issues related to their condition and treatment. Qualifications & Skills Master’s Degree in Social Work from an accredited institution (MSW required). Licensure such as LCSW/ACSW preferred; LMSW required in certain states. Preferable experience in health, human services, or healthcare settings. Strong interpersonal, goal-oriented, and relationship-building skills.
