Care Coordinator

Role: Full-time Care Coordinator supporting children aged 0–3 with developmental delays or disabilities within a family-centered, trauma-informed Early Intervention program. Responsibilities: Conduct functional assessments, develop and implement individualized service plans, coordinate services, support families through education and advocacy, facilitate team meetings, collaborate with community providers, and ensure accurate documentation. The role involves working closely with families, service providers, and internal teams to promote child development and family well-being. Qualifications: Bachelor's degree in Social Work, Child Development, or related field; experience working with families with special needs preferred. Valid driver's license, reliable transportation, and strong organizational and communication skills are required. Ability to work in varied settings, respond to crises, and manage flexible schedules. Work Environment: Community-based with travel, involving physical activity and exposure to diverse environments, including home visits and community settings.
