Care Coordinator

Role: Care Coordinator for the Birth to Three Program, supporting children aged 0–3 with developmental delays or disabilities and their families. Responsibilities Partner with families, service providers, and community resources to promote child development. Conduct functional assessments and develop individualized, family-centered service plans. Coordinate and facilitate services aligned with care plans. Support families through education, advocacy, and collaboration. Maintain accurate documentation and facilitate family-centered team meetings. Collaborate with internal teams and community providers. Manage a flexible schedule, including home visits and occasional evening or weekend hours. Meet productivity and service delivery expectations. Qualifications Bachelor's degree in Social Work, Child Development, or related human services field. Experience working with families with special needs preferred. Valid driver's license, reliable transportation, and ability to work in diverse settings. Strong organizational, communication, and documentation skills. Ability to respond effectively in crisis situations. Work Environment & Skills: Community-based role involving daily travel, physical activity, and working in homes and community settings.
