Provider Partnerships Manager

Responsibilities Build Referral Partnerships: Develop relationships with physicians, care coordinators, social workers, assisted living facilities, and senior-focused organizations. Own Territory Growth: Grow referral volume across your market by developing new partnerships. Operate in the Field: Spend 4–5 days per week meeting partners in person. Collaborate Cross-Functionally: Partner with clinical, operations, and patient experience teams. You're comfortable with range: You can give a presentation to 20 providers and walk into a solo PCP's office. Requirements You have empathy AND hunger. You've built relationships from scratch in high-activity sales/development roles. You're entrepreneurial and resourceful. You have exceptional attention to detail. You genuinely believe in the opportunity and mission.
