RN Case Manager - Hybrid Telehealth Home Infusion Specialist

Ponos Care in Chesapeake, VA, is seeking an RN Case Manager (TOC/Travel Infusion/RPM/LTSS) to provide hybrid telephonic case management and in-person infusion therapy, coordinating medical, behavioral health, and social support services across multiple states. You will conduct comprehensive assessments, develop individualized care plans, educate members and families, and ensure regulatory compliance (CMS/NCQA/HIPAA).
